Our clinics are made up of a multidisciplinary care team whose mission is to prevent or minimize (slow down) the progress of chronic kidney disease (CKD), and if necessary manage your diabetes, high blood pressure (hypertension), obesity among other conditions that may affect the health of your kidneys. Because of our services, starting dialysis treatment is prevented or delayed and/or, if necessary, educated about renal replacement therapy (RRT) and transplant.
